 If you are needing the required forms to enroll into the tribe, Please contact BIA and they will mail the CIB application to you.

 You will need;

1) Certified Birth Certificate.  No Copies

2) Social Security Card

3) Government Issued Identification (ie. Id, Driver's License, Passport) if over the age of 16.  If under the age of 16 parents identification will be required.
